IMPORTANT NOTICE  REGARDING YOUR DENTAL BENEFITS

  If you are having a dental procedure done with charges in excess of $300, it is highly recommended to have a “Dental Predetermination” submitted to the Carpenters’ or Floor Layers’ Fund. A “Predetermination” of benefits will let you and your dentist know before the work is started, exactly what will be covered and how much you (the member) will be responsible for when the work is completed.
